Information for Physicians
Whom Should You Refer?
- Individuals with dementia for whom you wish assistance with diagnosis and/or treatment
- Individuals with Alzheimer’s disease who are interested in research treatment opportunities
- Families requiring assistance caring for someone with dementia
- Individuals with isolated cognitive deficits (e.g. aphasia)
- Individuals with mild or questionable cognitive impairment
What Is the Referral Process?
- Fax your request for consultation to (416) 386 - 0458.
- You can download our referral form or use your own.
- Include patient ID, referral reason, past health, current medications.
- Fax relevant lab reports, scans, prior consults, hospital summaries.
After You Fax A Referral:
- We will fax you an appointment for your patient, along with instructions and a map.
- You will pass this information on to your patient or their caregiver.
- Please notify us if you are unable to reach your patient or if an alternate date is needed.
- We make reminder calls to patients in advance of their scheduled appointments.

PHYSICIAN EDUCATION:
We offer a variety of educational events for physicians and medical students wishing to increase their knowledge and practical skills. Some of these activities are described below. To inquire or sign up, contact us at (416) 386 - 9761.
Family Physician Workshops
We hold on-site half-day sessions, in which Family Physicians participate in case discussions and interactive problem solving on a variety of dementia related topics. Topics are selected to suit the interests and needs of each group.
Specialist Workshops
Same format as for Family Physicians but geared to the needs and interests of internists, geriatricians, neurologists and psychiatrists.
Rotations for Medical Residents or Fellows
We offer 4 to 12 week out-patient rotations for trainees in Geriatrics, Psychiatry, Internal Medicine, and Neurology.
Electives for Undergraduate Medical Students
Medical students have the opportunity to advance their clinical skills in brain-behaviour relationships through participation in 4 week to 12 week electives.
