Our Memory Clinic
Who Do We See?
Clinic services are covered by OHIP and require a physician referral. We assess and treat adults with impairment in memory, communication, and thinking. This includes individuals with Alzheimer’s disease, Lewy Body disease, frontotemporal dementia, and stroke, as well as individuals at increased risk of dementia due to family history or other predisposing factors. We do not accept referrals for learning disability, traumatic brain injury, or medicolegal consultation.

What Should You Bring To Your First Visit?
- Your Ontario Health Card
- A list of your medications or the actual pill bottles
- Scans or reports in your possession pertaining to your medical condition.
What Should You Expect When You Come to Our Clinic?
You will spend about an hour and a half with us at your initial visit. This may be split into 2 separate visits. Expect to be treated professionally and courteously. Our secretaries will assist you with checking in and with completing a questionnaire. You will have a photo taken for your medical chart. You will meet with a clinical assistant and one of our physicians. You will be interviewed and will undergo cognitive testing and a neurological examination. Results of your assessment will be discussed with you. Additional tests (e.g. a brain scan) may be ordered. Medications may be prescribed. You may be given an information sheet with suggested community resources. You will be given a follow up appointment as needed. Follow up visits last about 30 minutes.
